Title: Prognostische Wertigkeit von c-MET im klarzelligen Nierenzellkarzinom in Korrelation zum PD-L1-Status

Abstract: Aufgrund der schlechten Prognose des klarzelligen Nierenzellkarzinoms insbesondere im fortgeschrittenen/metastasierten Stadium ist die Untersuchung molekularer Marker zur Verbesserung des Therapieerfolgs von großem Interesse. Die Rezeptor-Tyrosinkinase c-MET, welche an Proliferation, Angiogenese sowie Metastasierung beteiligt ist, wird als prognostischer Faktor in verschiedenen Tumorentitäten diskutiert. In dieser Arbeit soll ihre prognostische Wertigkeit sowie die Eignung als Therapietarget im klarzelligen Nierenzellkarzinom untersucht werden. Außerdem stellt die Kombination von Cabozantinib, welches neben anderen Targets c-MET adressiert, und einem Immuncheckpoint-Inhibitor eine aktuelle Erstlinien-Therapieoption im metastasierten Stadium dar. In anderen Tumorentitäten wurde bereits eine c-MET abhängige PD-L1 Regulation beobachtet, sodass eine mögliche Korrelation dieser beiden Marker sowie deren gemeinsame prognostische Relevanz im klarzelligen Nierenzellkarzinom analysiert werden soll. Für weitere Erkenntnisse wird der Fokus abschließend auf die Expression der bisher nur wenig untersuchten Aktivierungsform p(hosphoryliertes)-MET gelegt. Die c-MET-Expression wurde mittels Immunhistochemie an formalin-fixierten und in paraffin eingebetteten Proben (FFPE) von 70 Patienten mit klarzelligem Nierenzellkarzinom, welche zwischen 2010 und 2021 partiell oder radikal nephrektomiert wurden, analysiert. Das Expressionslevel wurde durch den semi-quantitativ ermittelten H-Score festgelegt. Anschließend wurde unter Verwendung von Mann-Whitney-U und Kruskal-Wallis Tests eine mögliche Korrelation mit klinisch-histopathologischen Parametern statistisch geprüft. Sowohl der Median als auch der durch die Receiver Operation Characteristics (ROC) Kurvenanalyse ermittelte optimale Trennwert des H-Scores definierten zwei Gruppen für die Überlebensanalyse: c-METstark oberhalb und c-METschwach unterhalb des Trennwertes. Anhand des Log-Rank-Tests von Kaplan-Meier-Kurven wurde die prognostische Wertigkeit analysiert. Die univariate und multivariable Cox-Regression prüften den Einfluss der untersuchten Variablen auf das Überleben. Der Zusammenhang zwischen c-MET und den aus einer anderen Doktorarbeit stammenden Daten zur PD-L1-Expression wurde mittels Korrelationsanalyse bestimmt. C-MET war sowohl membranös (c-MET-m) als auch zytoplasmatisch (c-MET-z) exprimiert, wobei Ersteres höhere Expressionslevel aufwies. Es konnte eine Assoziation von c-MET-m mit klinisch-histopathologischen Parametern beobachtet werden. Das Stadium pT3 wies eine signifikant höhere Expression als pT1 und pT2 auf (p=0,010 und p=0,043). Das Auftreten von Metastasen war signifikant mit erhöhter c-MET-m-Expression korreliert (p=0,005), wobei Patienten mit synchroner und früh metachroner (≤24 Monate) Metastasierung gegenüber denjenigen ohne Metastasierung eine signifikant höhere Expression aufwiesen (p=0,007 und p=0,015). Nach Trennung durch die ROC-Analyse zeigten Patienten der Gruppe c-MET-mstark ein signifikant kürzeres Gesamt- (OS) und metastasenfreies Überleben (MFS) als die der Gruppe c-MET-mschwach (p=0,029 und p=0,001). In der multivariablen Analyse bestätigte sich dieser Marker als unabhängiger Risikofaktor für das metastasenfreie Überleben. Im Gegensatz dazu konnte der c-MET-z-Expression weder nach Trennung durch den Median noch durch den ROC-Trennwert eine prognostische Wertigkeit zugeschrieben werden. Wir fanden eine signifikante Korrelation zwischen c-MET-m- und PD-L1-Expression (Spearman Korrelation r=0,342; p=0,007). Die Kombination beider Marker ergab ein signifikant längeres Gesamt- und metastasenfreies Überleben bei Patienten mit c-MET-mschwach (gruppiert nach ROC) und PD-L1-Negativität, die Variablen c-MET-mstark/PD-L1+ und c-MET-mschwach/PD-L1+ waren in der multivariablen Analyse für das metastasenfreie Überleben mit einer signifikanten Risikosteigerung verbunden. Auch die kombinierte Betrachtung von c-MET-z (gruppiert nach ROC) und PD-L1 wies einen signifikanten Unterschied beim metastasenfreien Überleben auf, jedoch ohne Risikosteigerung in der Cox-Regression. Nach umfangreichen Etablierungsversuchen der p-MET-Immunhistochemie und Untersuchung möglicher Fehlerquellen ergab sich die Fixierung der FFPE-Präparate am ehesten als einschränkender Faktor. Eine spezifische Färbung gelang jedoch an kryokonservierten Gewebeschnitten. Da ein solches Präparat nicht von jedem Patienten des Kollektivs vorhanden und deren Qualität unzureichend war, konnte keine Analyse erfolgen. In dieser Arbeit wurde die membranöse c-MET-Expression als signifikanter negativer prognostischer Parameter im klarzelligen Nierenzellkarzinom identifiziert. Auch die Kombination mit PD-L1 ergab eine negative prognostische Wertigkeit. Unsere Daten bestätigen c-MET als ein geeignetes Therapieziel bei fortgeschrittener/metastasierter Erkrankung und unterstützen die kombinierte Adressierung beider Targets. Aufgrund der signifikanten Korrelation der beiden Marker müssen Signalweginteraktionen und daraus resultierend mögliche synergistische Effekte bei Ko-Inhibition von c-MET und PD-L1 weiter untersucht werden. Die verwendeten Antikörper gegen p-MET waren für unsere formalin fixierten Gewebe nicht geeignet. Weitere Untersuchungen sind nötig, um die bisher im klarzelligen Nierenzellkarzinom weitestgehend unbekannte Relevanz von p-MET zu ergründen. Die Erkenntnisse dieser Arbeit können dabei als Anhaltspunkt für Folgearbeiten dienen.
Due to the poor prognosis of clear cell renal cell carcinoma (ccRCC) especially in advanced disease, the examination of molecular markers plays a crucial role to improve the therapy efficacy. The receptor tyrosine kinase c-MET, which is involved in proliferation, angiogenesis and metastasis is widely discussed as a prognostic factor in various tumor types. We aimed to investigate its prognostic value and suitability as a therapy target in clear cell renal cell carcinoma. Moreover, one of the current first-line therapy options in metastatic disease is a combination of the tyrosine kinase-inhibitor Cabozantinib, which among other targets addresses the receptor tyrosine kinase c-MET, and an immune checkpoint-inhibitor. Considering a possible c-MET-dependent PD-L1 regulation as observed in other tumor entities, we examined a possible correlation between those two markers and their prognostic relevance in clear cell renal cell carcinoma. Finally, this thesis focused on the expression of the active form p(hosphorylated)-MET, that has been little studied so far. The expression of c-MET was analyzed by immunohistochemistry of clear cell renal cell carcinoma formalin-fixed paraffin-embedded (FFPE) samples from 70 patients who underwent partial or radical nephrectomy between 2010 and 2021. The protein expression level was defined by the semi-quantitative H-score and further correlated with clinico-histopathological parameters using Mann-Whitney-U and Kruskal-Wallis tests. For survival analysis, Receiver Operation Characteristics (ROC) curve analysis as well as the calculated median defined an H-score threshold for two patient groups: c-METstrong above and c-METweak below the cut-off. The prognostic value was investigated by Kaplan-Meier curves with log-rank-tests. The influence of the tested variables on survival was determined by univariate and multivariate Cox-Regression analyses. In a correlation analysis c-MET expression was set in relationship with PD-L1 expression, whose data were provided by another doctoral thesis. c-MET was found both on membrane (c-MET-m) and in the cytoplasm (c-MET-ct) with higher expression on membrane. Overall, we observed that c-MET-m was associated with clinical histopathological features. The stage pT3 showed significant higher expression than pT1 and pT2 (p=0.010 und p=0.043). Moreover, high c-MET-m expression significantly correlated with metastatic clear cell renal cell carcinoma (p=0.005) in which patients with synchronous and early metachronous (≤24 months) disease had significant higher c-MET-m expression than those without metastasis (p=0.007 and p=0.015). Patients with c-MET-mstrong score grouped after ROC-analysis had significant shorter overall (OS) and metastasis free survival (MFS) than those with c-MET-mweak expression (p=0.029 and p=0.001). C-MET-m was confirmed as an independent risk factor for metastasis free survival by multivariate analysis. In contrast, no prognostic value could be attributed to c-MET-ct expression in any test. We found a significant correlation between c-MET-m and PD-L1 expression (Spearman-correlation rho=0.342; p=0.007). The combination of both markers resulted in significantly longer overall and metastasis free survival in patients with c-MET-mweak (grouped after ROC) and PD-L1 negativity. The status c-MET-mstrong/PD-L1+ and c-MET-mweak/PD-L1+ were associated with a significant increased risk for the metastasis free survival in multivariate analysis. The combined consideration of c-MET-ct and PD-L1 also showed a significant difference in metastasis free survival, but without an increased risk in cox-regression. After extensive attempts to establish p-MET-immunohistochemistry and investigation of possible sources of error, the fixation of the FFPE samples proved to be the most limiting factor. However, a specific staining was successful on cryoconserved sections. The limited access to cryo sections and their inadequate quality did not allow analysis in this cohort. In this study, membranous c-MET expression was identified as a significant negative prognostic parameter in clear cell renal cell carcinoma. The combination with PD-L1 also revealed a negative prognostic value. Overall, our data confirmed c-MET as an appropriate therapy target in advanced/metastatic disease and support the combined addressing of both targets. As we found a significant correlation between c-MET and PD-L1, a possible signaling loop and a likely synergistic effect in co-inhibition of c-MET and PD-L1 has to be further investigated. The failed immunohistochemical examination of the p-MET expression indicates that the available antibodies cannot detect the antigens in our formalin-fixed samples. Further exploration is necessary to assess the unknown relevance of p-MET in clear cell renal cell carcinoma. The findings of this thesis can serve as informative basis for further investigations.
